Cognitive task analysis conducted to identify strategies that hospital and outpatient healthcare professionals use, and barriers encountered when they coordinate care during a medication safety incident involving an adverse drug reaction, drug-drug interactions, or drug-renal concern. Researchers conducted 12 interviewes with SMEs including physicians and pharmacists with implications for future efforts to enhance electronic health record designs for care coordination and to provide more formal guidance recaeding communication modalities.
